1. General practitioner
Registered with Meadow Medical Centre since March 2025. Out of hours: NHS 111 for advice.
2. Known medical conditions
Callum has no long-term medical conditions. He is on the ADHD pathway; an appointment took place in April 2025 and outcome documentation was delayed because forms were not completed by previous carers. Raised with the social worker, who is chasing.
3. Health chronology
- •15.07.25 EHCP now in place to support his needs in education.
- •23.12.25 Diagnosis of ADHD confirmed. Medication review booked at Northgate Hospital for 13.01.26.
- •02.01.26 GP visit following complaints of exzma like symptoms on his arm. Diagnosed with Keratosis Pilaris; emollient cream and antihistamine solution prescribed twice daily. Callum also raised a personal health concern but declined to be examined, stating he was not mentally prepared and asking to book again when he felt ready. Rebooked for 05.01.26. Skin condition to be followed up 26.01.26.
- •05.01.26 Attended. Examined, sample taken, topical treatment prescribed to be self-administered; the doctor explained how to apply it. Key worker session on personal hygiene the same day.
- •08.01.26 Callum began declining the treatment, saying the problem had resolved. To be booked for a check before the medication is disposed of.
- •09.01.26 Key worker session, personal hygiene follow up, covering his declining of treatment.
- •13.01.26 Attended Northgate Hospital for ADHD medication review. Engaged positively.
- •26.01.26 Declined the GP follow up on his skin condition. Rebooked for 06.02.26.
- •06.02.26 Declined the appointment. Rebooked for 14.02.26.
- •14.02.26 Attended. Advised an oil-based emollient as an alternative to the cream; he declined this option.
- •03.03.26 Thumb concern raised. Call made to arrange an appointment; after almost an hour on hold the decision was made to call again in the morning and monitor. Key worker session recorded.
- •04.03.26 Attended walk-in centre. Examined and supplied with pain relief to take as needed. X-ray arranged for the following day.
- •05.03.26 Returned for the x-ray. The x-ray team had left due to staffing. Re-examined; he reported discomfort only at rest. Agreed to return in the morning.
- •06.03.26 X-ray taken. A minor fracture was reported and a splint applied. Fracture clinic appointment arranged for the following day.
- •07.03.26 Attended fracture clinic. On examination the assessment was minor ligament damage rather than a fracture. Advised to sleep in the splint.
- •12.03.26 Follow up clinic. Advised to continue wearing the splint for two weeks. No sport during healing.
- •25.03.26 Review. Advised to continue the splint for one further week to complete healing.
- •31.03.26 Key worker session so Callum understood what to expect from an ECG.
- •01.04.26 Attended for ECG. The appointment was used to set expectations rather than carry out the procedure. Awaiting a letter for the actual appointment.
- •14.04.26 Letter received regarding an occupational therapy video call.
- •21.04.26 Occupational therapy video call. Thumb reviewed and reported as fine.
- •28.04.26 ECG attended. Positive engagement. Staff advised results would be sent to the ADHD team.
4. Allergies
Callum is allergic to blueberries.
5. Dental health
Name of dentist: Parkway Dental Practice. Surgery address: Hillside Dental Practice.
Registered since March 2025. Last check up November 2025; next due July 2026. Two fillings and one extraction at his appointment in June 2025. Staff to support Callum with brushing and educate around dental hygiene.
6. Optical needs
Sight test completed October 2025. No correction required. Next test due October 2027.
7. Hearing needs
Hearing reviewed 2023. No concerns raised at the time.
8. Emotional and mental health needs
Callum is on the ADHD pathway and awaiting the outcome of his medication review. No CAMHS referral is currently open.
9. Therapeutic intervention
Barnardo's sessions have been in place since September 2025 and are recorded as ongoing. The referral has been transferred to the Barton service.
Equestrian therapy weekly, arranged through the placing authority.
10. Immunisation and statutory medical
Immunisations up to date per the GP record. Last statutory medical 14.04.26; next due 14.04.27. Callum engaged with his LAC health assessment on 14.04.26.
11. Staff responsibilities
- •Staff to support Callum to attend all health appointments.
- •Staff to administer medication as prescribed and record administration.
- •Staff to record all health interactions in the health file.
- •Staff to escalate any concerns to the shift leader.
12. Other information
LAC nurse allocated. Young person is not assessed as competent to hold or self-administer medicine.
